How to Make an OMR Sheet (Free, Printable)

An OMR (optical mark recognition) sheet is an answer sheet where students fill in bubbles instead of writing answers. It makes multiple-choice tests faster to take and far faster to check. You don't need special software or pre-printed paper to make one: this guide walks you through creating a professional sheet with the free ToolAdda OMR sheet generator.

Step 1: Start from a preset

Open the generator and choose a preset that is close to your test:

School test: 50 questions with options A to D.
NEET-style: 180 questions in four subjects with +4 / −1 marking.
JEE Main-style: three subjects, each with multiple-choice and numerical-answer questions.
SSC, bank and UPSC-style: 100 questions with the usual option counts and negative marking.
True/False quiz and Survey for quick checks and feedback forms.

Every setting can be changed afterwards, so pick the closest one and adjust it.

Step 2: Add your header

Enter your school or institute name, the exam title and an optional subtitle such as the class, subject and maximum marks. Upload your logo in PNG, JPG, WebP or SVG format. Choose English, Hindi or bilingual labels to match your students.

The sheet colour changes the colour of lines, bubbles and labels. Red and blue sheets make students' dark pen marks stand out, while black prints perfectly on any printer.

Step 3: Choose candidate details

Field	When to use it
Roll number grid	Whenever you want the checker to identify students automatically. Choose as many digits as your longest roll number.
Booklet set code	When you give different question paper versions (A, B, C, D) to reduce copying.
Centre code	For tests held at several centres or branches.
Name, class, subject, date	Write-in boxes for information you'll read yourself.
Signature boxes	For formal exams where a candidate and invigilator should sign.

Step 4: Set up the questions

Each section has its own title, number of questions, question type and marks:

Multiple choice with 2 to 10 options labelled A B C, a b c or 1 2 3.
True / False with T and F bubbles.
Numerical questions, where students write and bubble a number of up to 6 digits.

Use sections for subjects (Physics, Chemistry…) or parts of a paper. Choose whether question numbers continue across sections or restart in each one. Enter the marks for a correct and a wrong answer; they're used by the checker for scoring and negative marking.

Step 5: Fit it on the page

The preview shows exactly what will print. If your sheet needs more than one page, try these in order:

Set Bubble size to Small. About 180 four-option questions fit on one A4 page.
Turn off the instructions box or fields you don't need.
Use US Legal paper if your printer supports it.

Multi-page sheets are fine too: each page has its own QR code, and the checker joins the pages of a sheet automatically.

Step 6: Add the answer key

Open Copies & answer key and click the correct option for each question, or paste the key as text such as ABCDDCBA… or 1-A, 2-C. If you use booklet sets, enter a key for each set. Tick Add answer-key pages to print a filled key for your records. The key is saved on your device and is picked up by the checker automatically.

Checklist before the exam

Print one sheet and check that all four black corner squares are fully visible.
Fill the test sheet with a few answers and try it in the checker.

Can I edit the sheet after downloading?

Yes. Your design is saved in the browser. Change it and download again. The QR code on the new sheet always matches its layout.

Can I make OMR sheets in Hindi?

Yes. Choose Hindi or English + Hindi labels. Institute names and titles can be typed in any language.

Is there a limit on copies?
